You will be given opportunity to detail your child’s medical conditions on your enrolment form. Should there be any changes to your child’s health, please complete the General Medical Advice Form. Management form and submit at the office.
If your child is required to take medication at school you must complete and return the Medication Authorityform.
If your child has an Asthma Plan please provide a copy from your GP to the office.
